Thank you for your interest in our Medicaid Product - EZ CARE. Below are the Provider Manual, the Cultural Competency Plan, and Pharmacy Drug List designed to offer you assistance for providing services to our EZ CARE members. Together we will provide EXCELLENT health services to our members and we look forward to a long lasting partnership. Effective August 22 there has been a change in the Medical Management and Eligibility and Benefit Verfication Telephone Lines for providers. Please click below on the "Provider Alert" for more information. In addition, beginning September 1, 2011 Florida Medicaid may not pay for a psychotropic medication prescribed for a child in the Medicaid program without the express and informed consent of the child's parent or legal guardian. As a provider of Medica Health Plans of Florida, Inc.'s Medicaid Plan, you are encouraged to participate in the Vaccines for Children Program (VFC) as administered by the Department of Health, Bureau of Immunizations and ensure an adequate supply of vaccines at all times. Title XXI MediKids enrollees do not qualitfy for VFC program. You must bill Medicaid fee-for-service directly for immunizations provided.
